Not sure it is PS, besides handmade drawings that are later coloured digitally Par Olofson also paints in oil.
The actual idea and execution are good from a technical point of view but the character design is what leaves a lot to be desired.

One of the worst covers that I've seen recently was for Blessed in Sin's "Honor the Anus of Mary". It was up for auction on eBay. I get the idea of it but nevertheless, it's ghastly artwork, hahaha.
