Not an album cover, but this has to win some sort of "lack of originality" award:
It's literally just a combination of the Xasthur and Leviathan logos.

I tend to dislike Ed Repka works because they're too corny and cartoony. I could think of many better ways to portray an album called "Evil Dead - Annihilation Of Civilization", but it's cool and well done none the less. Maybe it fits well to the crossover style they had after all. Vio-Lence was kinda cool as well.

I would assume the band or label gave Ed Repka an idea of what they wanted for the art. Besides what the end result was, he's still a great artist. |
