Although I don't think BT ripped off the artwork, Games Workshop did it specifically for them and Realm of Chaos is actually the soundtrack to that game.
That is correct, both Bolt Thrower and Earache had permission to use the Warhammer artwork and Ian Miller's Illustrations for the Realm of Chaos LP.

Warloghe also used another Ian Miller piece for their black tower demo.

Cloven Hoof Dominator was done by Jon Blanche of Games Workshop, as was Sabbat- A History Of Time To Come. The list goes on...........

Well, his origins are in Krynn, but he ended up in Ravenloft in "The Knight of the Black Rose" novel. He and Strahd beefed pretty hard, if I remember correctly.

He is, so far i know, the only character throwing out of ravenloft. His
companion, the weredax take over the realm of him. But it was pretty
lame. I think were over 2 books. Real issue was the copyright of the
figure weren't any longer avaivle for Ravenloft Supplies. His name is
also erased in newer Ravenloft books.
